1. Color Photo:
If there is not already a photo in your child's gotsport account. Please upload a color head shot (passport style).

• Select Child's Name
• Upload color head shot. no hats or sun glasses. No full bodied action shots.

2. Copy of Birth Certificate or Passport:
If your child's date of birth has NOT already been approved in gotsport and/or your child is new to EPYSA TRAVEL soccer. Please upload a copy of your child's birth certificate or passport in gotsport, or email a copy to your club registrar.

• Upload a copy of birth certificate or passport under EPYSA Birth Certificate from the drop down menu

3. Eastern Pennsylvania Youth Soccer Player/Participant Waiver of Liability 25/26:

  • Go to Family
  • Select Child's Name
  • Click on Dashboard
  • Scroll down to Requirements
  • Complete the Eastern Pennsylvania Youth Soccer Player/Participant Waiver of Liability 25/26 by clicking on "details" to the right of the requirement

4. International Transfer Clearance (ITC):

Was the player born or played outside of the US? Please contact your club registrar for more information. The player may need International Clearance from US Soccer if he or she has not already received it previously. Be advised that the international clearance process can take a couple of weeks to approve. 

5. 2025-26 PA Classics Medical Release:
Please sign and turn in to your head coach.

6. SafeSport: 18 Years Old or turning 18 During the 2025-26 Season?
Safe Sport Training for the 25/26 season will be required to be completed by any player who is 18+ or will turn 18 years old during the 25/26 season prior to a player being able to be added to a team’s league roster.

Safe Sport can be accessible in the player’s GotSport Account on their dashboard page. Please see below for helpful instructions on how a player can complete Safe Sport in GotSport.

If anyone is having problems finding a form, please try logging out and then back in to gotsport. You can also try a different device or browser.
