EPYSA Teams (Academy, NAL, Elite, Premier, United)

 

1. Color Photo: If there is not already a photo in your child's gotsport account. Please upload a color head shot (passport style). If you would like your child's photo updated or if you are having problems uploading it yourself please email a photo to club registrar at mklein@paclassics.org and sure to label it.

- Log in to your gotsport account: https://system.gotsport.com/
If you do not already have an account, please reach out to the club registrar at mklein@paclassics.org, so that one can be created for you.

- Go to Family

- Select Child's Name

- upload color headshot. no hats or sun glasses. no full bodied action shots.

 

2. Copy of Birth Certificate or Passport: If your child's date of birth has NOT already been approved in gotsport and/or your child is new to EPYSA TRAVEL soccer. Please upload a copy of your child's birth certificate or passport in gotsport, or email a copy to club registrar at mklein@paclassics.org .

- Log in to your gotsport account: https://system.gotsport.com/
If you do not already have an account, please reach out to the club registrar at mklein@paclassics.org, so that one can be created for you.

- Go to Family

- Select Child's Name

- Go to Documents

- upload a copy of birth certificat eor passport under EPYSA Birth Certificate from the drop down menu

 

3. Eastern Pennsylvania Youth Soccer Player/Participant Waiver of Liability 24/25:

- Log in to your gotsport account: https://system.gotsport.com/
If you do not already have an account, please reach out to the club registrar at mklein@paclassics.org, so that one can be created for you.

- Go to Family

- Select Child's Name

- Click on Dashboard

- Scroll down to Requirements

- Complete the Eastern Pennsylvania Youth Soccer Player/Participant Waiver of Liability 24/25 by clicking on "details" to the right of the requirement
 

4. International Transfer Clearance (ITC) Questionairre:

Was the player born or played outside of the US? Please contact Club Registrar, Michelle Klein, at mklein@paclassics.org for more information. The player may need International Clearance from US Soccer.

 

5. 2024-25 PA Classics Medical Release: Please sign and turn in to your head coach.

6. 18 Years Old?: SafeSport Training Certificate

 

If anyone is having problems finding a form, please try logging out and then back in to gotsport. You can also try a different device or browser.
